What to bring to a first visit
The more history you bring, the more specific the first conversation can be. Nothing on this list is required to make the appointment.
- Photo ID and insurance card
- Current medication list, including over-the-counter medicines and supplements
- Recent lab results, if you have them
- Referral information or records from your primary care provider, if applicable
